Conditions for Purchase of Hearing Aids

By making a purchase from Century hearing I agree that:

1. I have been medically evaluated within the the last 6 months and have been determined to be a candidate for a hearing instrument. I agree to provide the hearing test results as part of purchase from Century Hearing for device programming, OR

2. I acknowledge that I am eighteen (18) years of age or older and have been advised by Century Hearing that the Food and Drug Administration (FDA) has determined that my best health interest would be served to have a medical evaluation by a licensed physician, preferably a physician who specialized in diseases of the ear, before purchasing a hearing instrument; or tested by a licensed audiologist utilizing established procedures in the fitting of hearing aids. However, I do not wish either a medical evaluation or test before purchasing a hearing aid from Century Hearing.
